Nam Sai Gon International General Hospital recently received D.S., a 55-year-old male patient from Cambodia, who was admitted in a condition of severe headache and repeated vomiting. He was diagnosed with hemorrhagic stroke and had been treated at a local hospital for five days, but his condition did not improve.

Successful Emergency Treatment of Hemorrhagic Stroke for a Cambodian Patient

After initial examination and assessment, doctors at Nam Sai Gon International General Hospital ordered a CT scan to accurately identify the cause of the stroke. The patient’s brain CT results showed a left hemispheric intracerebral hemorrhage measuring 35 x 65 x 80 mm, with surrounding cerebral edema and additional bleeding into the third ventricle and both lateral ventricles.

Dr. DO ANH VU, MMed, Specialist Level II, Neurosurgery & Spine Department, explained that worldwide, hemorrhagic stroke accounts for about 15% of all stroke cases. In stroke management, taking advantage of the “golden window” can significantly improve treatment outcomes.

However, in the case of patient D.S., who was transferred from Cambodia in critical condition — after five days of ineffective treatment at a local hospital — the Neurosurgery team at Nam Sai Gon International General Hospital decided to perform hematoma drainage combined with intralesional thrombolytic therapy.

Hematoma Drainage Combined with Fibrinolytic Therapy – A Minimally Invasive Surgical Technique Offering High Effectiveness for Cerebral Hemorrhage Patients

“The technique of draining the hematoma combined with fibrinolytic therapy is a minimally invasive surgical method. It requires only a small burr hole in the skull, opening of the dura, and insertion of a catheter into the hematoma under the guidance of a 3D Navigation system, allowing the hematoma to be drained effectively.

This is one of the most advanced surgical techniques for treating intracerebral hemorrhage, and only a few medical facilities in Vietnam are capable of performing it. Unlike traditional open craniotomy—which requires a large skull opening, a long incision, higher blood loss, a greater risk of rebleeding, multiple complications, and prolonged recovery—this minimally invasive method offers significant advantages,” Dr. Vu added.

Contrast-Enhanced CT Images Before and After Surgery

After undergoing minimally invasive surgery using 3D Navigation, the patient recovered remarkably quickly. Following three days of drainage and a repeat CT scan, only a minimal amount of hematoma remained. The patient was fully alert, speaking normally, and had regained almost complete motor function in his limbs. After seven days, he was discharged and continued physical therapy at home.
