Mr. P.D., 40 years old, from Cambodia, suffered a lower limb fracture 15 years ago that significantly impacted his daily life. Unfortunately, a recent accident worsened his condition, prompting him to seek treatment at Nam Sai Gon International General Hospital.
Upon admission, the medical team immediately conducted a clinical examination and necessary diagnostic imaging. An orthopedic case conference was held to determine the most appropriate surgical intervention.

Imaging and examination revealed a double-level fracture of the left fibula and a 20-degree angular deformity of the left tibial shaft. Given the patient’s wish for full anatomical and functional restoration, the orthopedic team — Dr. Vi Van Duong, Specialist Level I, Dr. Nguyen Van Huynh, and Dr. Dao Danh Dung — agreed on a surgical plan: Osteotomy with realignment, intramedullary nailing, and iliac crest bone grafting to reinforce the repair.

The 2-hour surgery was successfully completed. Postoperative evaluation showed stable vital signs, a dry surgical site, and no vascular complications. Mr. P.D. was closely monitored for recovery progress and any signs of postoperative complications. By day 7, his condition had stabilized, and he began in-bed physical rehabilitation to prevent deformities and support early mobility. He was discharged soon after.

“Tibial shaft axis deviation is a common complication following improperly healed fractures”, explained Dr. Vi Van Duong, Specialist Level I. “When a malunion occurs, the limb can become shortened or misaligned, which significantly affects the patient’s ability to bear weight and perform daily activities”.
The degree of misalignment must be evaluated through clinical exams, axis measurements, and advanced imaging. If the deformity has minimal impact on function, surgery may not be necessary. However, in severe cases like Mr. D’s, realignment surgery is essential to restore mobility and quality of life.

The Orthopedics & Traumatology Department at Nam Sai Gon International General Hospital places strong emphasis on post-discharge follow-up to monitor recovery and guide ongoing rehabilitation. Mr. D. is scheduled to return for evaluation one month post-op to assess the surgical outcome and detect any potential late complications.
“Early detection of hidden issues after surgery greatly increases the success rate of treatment”, added Dr. Duong.
Tibial and fibular fractures are common injuries from everyday accidents or trauma. Dr. Duong advises that patients should not delay seeking care at a facility with advanced diagnostic equipment and orthopedic expertise, as early intervention is crucial to avoid long-term complications.
